CASE STUDY SCENARIO

Mr. McLaren is a 48-year-old man who is admitted to the emergency department (ED) with abdominal pain and general weakness.  He reports that he drinks “one bottle of scotch whisky every week for the past one year, and one to two cans of beer during the weekends.”  For the past three months, his condition has started to deteriorate. His wife claims that “he has not been eating well for the past three months”.  Mr. McLaren reports he has passed dark red stool on three occasions – the last one being  just one day before coming to ED. On examination, Mr. McLaren has an enlarged abdomen with tenderness upon palpation.  He also presents with the following clinical manifestations: peripheral edema, pale yellow skin color, dry mucus membranes and body weakness. The physician suspects that Mr.  McLaren has hepatic cirrhosis.  His vital sign findings include: Temperature 37.8 Celsius, BP 94/52, Pulse 110, Oxygen saturation 94%.  From your nursing classes, you know that some of the treatments Mr. McLaren may receive are paracentesis, ultrasound and CT scan of the abdomen, as well as vitamin K administration.

PART 1: Questions

  1. Based on the patient’s diagnosis and the examination data, what potential complications are suspected for the patient to have and what are the possible treatments for these complications? Explain your answers. (list at least two potential complications)
  2. What additional information would you like to obtain regarding the bleeding episodes? Explain your answer.
  3. What laboratory work or diagnostic tests would you like to investigate in this patient? Why?
  4. McLaren is going to undergo a paracentesis to relieve the ascites. What are 3 nursing considerations you need to make for a patient undergoing a paracentesis?
  5. What kind of diet is recommended for the patient while he is at the hospital? Explain your answer.
  6. Discuss at least 3 initial nursing interventions that you would start for the patient. Explain your rationales for each nursing intervention.

PART 2: Nursing Care Plan

Good Luck
