Why Might These Issues Be Barriers To Homecare And Hospice Delivery?
The issues might be an obstacle to hospice and home care delivery in many ways. First, the Center for Medicaid Services and Medicare (CMS) requires physician assistants and nurse practitioners to validate certificates of medical necessity which are needed to file an entitlement for health equipment that is used for home care. Secondly, these centers prohibit the above practitioners from endorsing home care delivery which according to CMS only physicians are authorized for these services.
Improving Home Health and Hospice Services
Physician’s assistants, nurse practitioners, clinical nurse specialists, and certified nurse-midwives are progressively providing essential health services to the beneficiaries of Medicare, particularly in the countrysides. Since the above professionals are more familiar with such areas, it will be more appropriate to use them to sign orders. They are also more available as compared to physicians ensuring the service delivery to the beneficiary is uninterrupted. They should, therefore, be allowed to certify the suitability of home health and hospice services.
Financing the Services
Hospice and home health services can be funded by developing a payment adjuster for beneficiaries with high-cost services. For example, payments should be increased on the last and first day of service to patients who require additional care and attention. With this, the hospice will remain financially stable even if patients are referred to them at costly stages. Financial incentives should also be provided to nursing homes to provide home health and hospice services which can be achieved by coming up with a distinct hospice benefit for residents in nursing homes by allowing such homes to bill Medicaid for residents on hospice directly.
